There are two main types of esophageal cancer: Squamous cell carcinoma, which originates in the flat cell lining located in the upper part of the esophagus, and adenocarcinoma, which develops in the cells of the lower esophagus, often near the junction with the stomach. Esophageal cancer is known for its aggressive nature and its early detection challenges.

A chronic irritation of the esophagus might contribute to the development of esophageal cancer. Thus, several factors can provoke these changes, such as:

  • Gastroesophageal Reflux Disease (GERD) – Chronic acid reflux may cause irritation and inflammation of the esophagus. Over time, this can lead to cell changes, increasing the risk of adenocarcinoma.

  • Tobacco and alcohol consumption – Heavy and prolonged use of tobacco and alcohol increases the risk of esophageal cancer. These substances can damage the cells lining the esophagus, potentially resulting in the development of cancerous cells.

  • Obesity – Excess body weight, especially around the abdomen, increases the risk of developing esophageal adenocarcinoma.

  • Dietary factors – Diets high in processed foods and low in fiber, fruits, and vegetables are associated with an increased risk of esophageal cancer.

  • Barrett’s Esophagus – This condition, where the lining of the lower esophagus changes due to acid reflux, significantly raises the risk of adenocarcinoma.

Though it is challenging to recognize and diagnose this disease in its early stages, many of the most notorious symptoms appear until it has progressed. These esophageal cancer symptoms are:

  • Dysphagia – Difficulty swallowing, especially solid foods.

  • Unintended weight loss – Rapid and unexplained weight loss.

  • Persistent indigestion or heartburn – Chronic or severe acid reflux symptoms.

  • Chest pain – Discomfort or pain behind the breastbone.

  • Chronic cough – Persistent coughing is often accompanied by hoarseness.

  • Regurgitation – Bringing food or fluids back up.

The choice depends on the cancer’s stage, type, and the patient’s overall health. Treatment options include:

  • Surgery – Surgical removal of the tumor or part of the esophagus may be an option for localized cancers.

  • Radiation therapy – High-energy rays target and destroy cancer cells, often used before or after surgery.

  • Chemotherapy – Anti-cancer drugs are used to slow or kill the growth of cancer cells.

  • Targeted therapy – Drugs targeting specific molecules involved in cancer growth may be recommended.

  • Immunotherapy – This treatment boosts the body’s immune system to fight cancer cells.

Esophageal cancer is commonly diagnosed at an advanced stage, affecting the prognosis. Survival rates vary based on cancer type and stage—generally, the earlier the diagnosis, the better the forecast. However, advancements in treatments have improved outcomes for some patients. Supportive care, including nutritional counseling and pain management, is key to enhancing the quality of life for those with esophageal cancer.
