💼 About the Role

Signature Health Services is hiring a Full-Time Physical Therapist to join our trusted and compassionate home health team. This is more than just a job — it’s an opportunity to do meaningful work in a role that offers freedom, flexibility, and full support.

We use WellSky/Kinnser EMR and provide comprehensive onboarding for all documentation workflows, including OASIS Start of Care, Recertification, and Resumption of Care visits.

📋 Role Expectations

  • Provide in-home PT care, covering Baytown and surrounding counties
  • Document evaluations, OASIS assessments, care plans, and visit notes in WellSky
  • Manage a blended caseload of 30–34 visits/week (adjusted based on acuity and visit type)
  • Coordinate with nurses, aides, and other rehab disciplines

Blended Visit Model

Productivity accounts for the complexity of visit types. Time-consuming visits like Start of Care or Recerts will reduce total visit count expectations so you can focus on quality.

  • Texas-licensed Physical Therapist
  • Valid driver’s license and clean driving record
  • Strong documentation skills (or willingness to learn with full support)
  • Home health experience preferred but not required

What Does a Physical Therapist Do?

As part of our home physical therapy services, our physical therapists:

  • Conduct thorough patient evaluations
  • Create personalized care plans tailored to each patient’s needs
  • Implement exercises and techniques to improve strength, balance, and mobility
  • Monitor patient progress and adjust therapy plans as needed
  • Educate patients and their families on exercises and care between visits
  • Collaborate with nurses, doctors, and other healthcare team members

Essential job functions of a PT

Provide a clean, safe, and comfortable environment. Promote respect for patient privacy and property.

Proactively adheres and commits to Agency’s vision, philosophy, core values and code of conduct.

Provide effective communication to patient and family members and other health care professionals.

Accurately and appropriately, report changes and document pertinent information and care rendered to patient to ensure continuity of care.

What Certifications or Degrees Do I Need to Become a Physical Therapist?

To work as a home physical therapist with us, you need:

  • A Doctor of Physical Therapy (DPT) degree from an accredited institution
  • A valid state license to practice physical therapy
  • CPR certification (current)
  • At least one year of clinical physical therapy experience (home health experience preferred but not required)

Bonus points if you have experience in home physical therapy or a strong desire to work in physical therapy home health environments. We provide mentorship and ongoing education to support your professional growth.

Physical and Mental Effort of a PT

Prolonged standing and walking is required, including ability to lift up to 50 lbs. and move patients. Requires working under some stressful conditions to meet patient needs, patient/family individualized psychosocial needs, meet deadlines, make quick decisions. Ability to utilize durable medical equipment in the home. Requires hand-eye coordination and manual dexterity.
